While choosing to avail the consultation service on the MyFollowUp platform you understand, accept and agree to the following:

• Personal follow up is the best option for medical treatment and MyFollowUp will have limitations as it is not a substitute for advise based on physical checkup.
• You are completely responsible for choosing your own doctor or health care provider and MyFollowUp shall not be held responsible or liable for the selection of such doctor or health care provider;
• Please note that MyFollowUp does not recommend or endorse any doctor or health care provider mentioned in our Application
• MyFollowUp does not make any representations or warranties with respect to these doctors or health care providers or the quality of the healthcare services that they may provide.
• MyFollowUp shall not be liable for the services or advice or prescriptions provided by the health care professional or doctor, and MyFollowUp bears no liability for the consequences to you, by your use of the doctor Services.
• Information regarding doctors and health care providers and practice information found in our Application is intended for general reference purposes only. Such information found on the Application is self-reported by the doctor or the health care provider and this information may change and become out of date or inaccurate.
• MyFollowUp does not provide any advice or qualification certification about any particular doctor or health care provider. You are encouraged to independently verify any such information you see in the Application with respect to a doctor or health care provider that you seek to make an Applicationointment with.
• MyFollowUp will refund service charges only if the doctor declines to accept your call on our Application.
• You as a user of this Application are 18 years of age or older and of sound disposing mind and that your use of our Application shall not violate any Applicationlicable law or regulation. You will be responsible for advise sought for a patient less than 18 year of age.
• MyFollowUp undertakes not to share your personal medical data with any third party, but reserves the right to use this data for analytical and research purposes.
• All information you enter at the time of registration is true and correct, that there is no concealment and you agree to keep it updated and current.
• Your registration in our Application is solely for your personal and non-commercial use. Any use of this Application or its content other than for personal purposes is prohibited.
• All information, content and material contained in MyFollowUp is intellectual property and hence covered by all relevant laws on the subject. All trademarks, services marks, trade names, and trade dress are proprietary to MyFollowUp. No information, content or material from the Application may be copied, reproduced, republished, uploaded, posted, transmitted or distributed in any way without the express written permission of MyFollowUp.
• MyFollowUp is only a service provider and not responsible for any legal, moral, or ethical consequences/claims arising from any medical consultation on this Application. You hereby indemnify, defend, and hold MyFollowUp and its representatives and other authorized users harmless from and against any and all losses, damages, liabilities and costs arising from your use of our Application.
• MyFollowUp does not warrant that the functions, content, information and materials on the Application and / or Services, including, without limitation any third party sites or services linked to the Application will be uninterrupted, timely or error-free.
• Any dispute or controversy arising under or in connection with the use of this Applicationlication will be settled in accordance with relevant laws through arbitration with the Applicationointment of a mutually agreeable neutral third party as arbitrator. Cost and fees of arbitration would be shared equally by all parties.
• Parties agree to have Delhi jurisdiction for any dispute resolution necessitated through courts.
• Above Terms and conditions above survive the termination or expiration of medical consultation through this Applicationlication.

Doctors Terms and Conditions

While choosing to provide medical consultation service on the MyFollowUp platform you understand, accept and agree to the following:

• That you certify that you are an authorised medical practitioner and hold a valid practice licence which you have uploaded on our Application.
• You certify that all information that you have uploaded about yourself is correct and true, and no element thereof has been misrepresented or concealed. That you alone shall be responsible for any legal liability arising from the information which you have uploaded on the Application.
• Please note that MyFollowUp does not recommend or endorse any client, third party or service providers linked to this Application.
• Information provided by any patient, third party or service linked to this Application is intended for general reference purposes only. Such information found on the Application is self-reported by the information. MyFollowUp is not responsible for the contents or the veracity of such information.
• MyFollowUp undertakes to refund consultation fee to the patient in case you decline to accept his/her call on our Application. You would not be entitled to fee for any such eventuality.
• All information, content and material contained in MyFollowUp is intellectual property and hence covered by all relevant laws on the subject.
